A training attendance sheet in English and Spanish.

For any class, from a new hire orientation to a forklift refresher. Headings are in both languages so the whole crew can fill it out. Print the sheet or save it as a PDF.

How to use it

Three habits that make it worth filling out.

  1. Step 1

Fill in the top before the class

Topic, trainer, date, length and place. A sheet with no topic can't show what anyone was trained on.

  2. Step 2

Have each person sign for themselves

No one signs for someone else. Anyone who leaves early or misses part of the class gets a note.

  3. Step 3

File it with the training record

Keep the signed sheet with the rest of the training records. If the class had a test, record who passed.

Do this on a phone instead of paper.

Sign people in on a phone, record who passed the check, and keep it with each person's training record.

Common questions

Why keep a training attendance sheet?
It is your proof that a person was in the room for a topic on a date. Some OSHA standards ask for training records, and an inspector or an insurer may ask to see them.
Does a signature prove someone understood the training?
No. It shows they attended. If the training matters, add a short quiz or a hands-on check and record who passed. The last column is for that.
Can I change the Spanish on this sheet?
Yes. Download it as Word and change the headings to match how your crew says things.
