A confined space entry permit that lists what the rule asks for.

OSHA 1910.146(f) lists what an entry permit has to identify. This form has a place for each item. Fill in a permit for each entry and post it at the entry. Print the sheet or save it as a PDF.

How to use it

Three habits that make it worth filling out.

  1. Step 1

Fill it out before anyone enters

Complete every item, run the tests, and have the entry supervisor sign. A permit with blanks hasn't documented the preparations.

  2. Step 2

Post it where entrants can read it

The completed permit goes at the entry portal, or somewhere equally clear, so entrants can confirm the preparations were done.

  3. Step 3

Cancel it when the job is done

Cancel the permit when the work is finished or when something comes up that the permit doesn't allow. Note any problems so the program can be fixed. Keep it for at least a year.

Do this on a phone instead of paper.

Fill this out on a phone, with photos and signatures, with no signal needed. The record is kept for you and nothing gets lost in a truck.

Common questions

What does a confined space entry permit have to include?
OSHA 1910.146(f) lists it: the space, the purpose, the date and duration, the entrants, the attendants, the entry supervisor, the hazards, how they are controlled, the acceptable conditions, the test results, rescue and emergency services, communication, equipment, any other permits, and anything else needed for safety.
How long do I keep a canceled permit?
At least one year, so the permit-required confined space program can be reviewed (1910.146(e)(6)). Note any problems found during the entry on the permit.
Is this for construction?
Construction confined spaces fall under a different rule, 29 CFR 1926 Subpart AA. Check that rule for the permit it asks for. This template follows the general industry rule.
