Know which forklift operators are due for evaluation.

Add each operator and the date of their last evaluation. The tracker works out the next one, three years on, flags anyone coming due or overdue, and lets you mark an operator who needs a refresher sooner.

How the dates work

  • The rule asks for an evaluation of each operator at least once every three years. The tracker adds three years to the last evaluation date.
  • A forklift certification doesn't expire on a printed date in the rule. The three-year evaluation is the clock, and your own program can set a shorter one.
  • Refresher training is due sooner after unsafe operation, an accident or near miss, an unsafe evaluation, a different truck type, or a change in the workplace. Tick Refresher on that operator.

The Excel version

  • The Excel file does the same with formulas, and adds columns for the refresher trigger, the trigger date and the date the refresher was done. Dates turn red when overdue and amber in the last 90 days.
  • The list on this page stays in your browser. Nothing is sent, and it sends no reminders.

Run this in the field.

PortalBase tracks each operator's skill check and certificate, and alerts the operator and the supervisor before the date passes.

Common questions

How long is a forklift certification good for?
The rule doesn't give the certificate an expiry. It asks for an evaluation of each operator's performance at least once every three years, and refresher training in the cases it lists (1910.178(l)(4)). Many employers treat the three-year date as the renewal.
Does an accident or near miss reset the three-year clock?
It triggers refresher training, including an evaluation of that training. Whether you also restart the three-year count is up to your program. Write down what you decide and apply it the same way every time.
Where do I keep the evaluation records?
The employer certifies each operator's training and evaluation with the operator's name, the dates and who did them. Keep that certification where you can show it. The evaluation card maker on this site makes one.
