A corrective action log that shows what is still open.

Every finding gets a line, an owner and a date. The Excel version counts the days open and flags what is overdue. Print the sheet or save it as a PDF.

How to use it

Three habits that make it worth filling out.

  1. Step 1

Log it the day you find it

Write what was found, how serious it is, what will fix it and who owns it. A finding with no owner doesn't get fixed.

  2. Step 2

    Set a real date

A due date a person can meet beats a date that gets moved every week. Give high findings the shortest dates.

  3. Step 3

    Review the open lines weekly

Read down the lines with no closed date. Overdue ones go to the person who can clear the block. Close a line only after someone checks the fix.

Do this on a phone instead of paper.

Report incidents and near misses from the phone, with the investigation and the corrective actions kept together on one record.

Common questions

What is a corrective action log?
A running list of problems found in inspections, incidents and audits, with the fix, the person responsible, the due date and the date it was closed. It shows at a glance what is still open.
What goes in a corrective action?
What was found, how serious it is, what will be done, who owns it, when it is due and when it was closed. If the fix is a change to a procedure or a training, say so, so it can be checked later.
Is a corrective action the same as a repair?
Not always. A repair fixes a thing. A corrective action fixes the cause, which might be a repair, a new procedure, a training or a change to the work. Many findings need both.
