An OSHA 300 log you can print or fill out in Excel.

One line for each recordable case. The Excel file adds up the columns and fills in a 300A summary sheet, with a 301 sheet and a privacy case list. It isn't OSHA's form, and it can't be uploaded to OSHA's site as it is.

How to use it

Three habits that make it worth filling out.

  1. Step 1

Enter each case within seven days

When you learn a recordable injury or illness has happened, put it on the log within seven calendar days. Not sure it counts? Run it through the recordable case helper first.

  2. Step 2

    Keep the days current

If a case is still open at year end, update the day counts when it closes. Change an old entry by lining it out and entering the new facts.

  3. Step 3

Total it at year end and make the 300A

Add up each column, check the log for mistakes, and use the totals for your 300A summary. The Excel version does the adding. A company executive certifies the summary before it is posted.

Do this on a phone instead of paper.

PortalBase builds the 300, 300A and 301 from your incident reports, with PDFs and the CSV for OSHA's online filing. You review it, and you file it yourself.

Common questions

What goes on the OSHA 300 log?
One line for each recordable case: the case number, the employee, the job title, the date, where it happened, what happened, how serious it was, the days away or restricted, and the type of injury or illness. Each case also needs an incident report. OSHA's 301 form is one way to do that.
How long do I keep it?
Five years after the end of the calendar year the records cover (1904.33). You also update the stored logs if a case changes during that time.
What is a privacy case?
For some injuries, such as one to a private body part, the rule says to write privacy case in place of the name on the log. You keep a separate confidential list of case numbers and names (1904.29). Check the rule for which cases qualify.
Can I upload this file to OSHA?
No. This is a working log to print or fill out. OSHA's online filing takes its own file format. PortalBase doesn't file with OSHA for you, and neither does this template.
