Write down who got what, the size or ID and the condition it was in. The worker signs for it. Print the sheet or save it as a PDF.

Free to print, copy and change. It’s a starting point. Add the lines your equipment and your rules need.
How to use it
Three habits that make it worth filling out.
Step 1
Log it at the handoff
Write the line when the gear changes hands. Not at the end of the week.
Step 2
Use an ID for gear that gets reused
Harnesses, SCBAs and fit-tested respirators should have a serial number or tag, not just a size.
Step 3
Note replacements
If gear is damaged, expired or worn, write it and replace it. Pull it from use. Don't hand back a bad one.
Do this on a phone instead of paper.
Log PPE handouts on a phone, with the worker signing on the screen. It works with no signal and the log is kept for you.

Common questions
- Why log PPE issuance?
- It shows who has what, when it was handed out and what shape it was in. When something is recalled or expires, the log tells you who to find.
- Does the log show the worker was trained on the PPE?
- No. A signature shows they got it. Training on how to use and care for it is a separate record. See your company procedure and the current PPE rule, OSHA 29 CFR 1910.132.
- Does the worker pay for the PPE?
- Rules differ by item and by situation. Check the current rule and your own policy. The log just records what was issued.
